My name is Daniel Walzman. I am a neurosurgeon at Northern New Jersey and currently work with North Jersey Brain and Spine Center. And I also serve as chief of Endovascular Neurosurgery at Hackensack University Medical Center.So an ischemic stroke is most often caused by a clot we typically would call an embolism. It is a clot that most often starts somewhere else in the body and travels to the brain. The most common site for the embolus originating is the heart. Another somewhat common cause is a blockage in the carotid artery in the neck. These clots travel to the brain and block a normal artery in the brain and when the artery in the brain is blocked that region of the brain has insufficient blood flow.
There are different types of ischemic stroke. There is a thrombotic ischemic stroke or an embolic ischemic stroke. But the thrombotic type of ischemic stroke which is really caused by narrowing in the artery in the brain that is very rare, as opposed to the embolic stroke that is much more common.
Subarachnoid hemorrhage is the type of bleeding you get typically from a ruptured aneurysm in the brain. An aneurysm of the brain represents a small outpouching of the vessel. That represents a weak point in the vessel. And it can be prone to rupture. When an aneurysm ruptures, it causes bleeding around the brain in the subarachnoid space, and therefore referred to as a subarachnoid hemorrhage.
So hypertensive bleed is a bleed in the brain usually in the deeper portions of the brain, often in the thalamus or the globus pallidus putamen which is called the basal ganglia. It is a bleed that is caused by the breakdown of very small tiny blood vessel within the brain. And the breakdown of the vessels thought to be predisposed by chronic high blood pressure, causing chronic damage to these small blood vessels. And when the blood vessels weaken you have an acute spike in the blood pressure that will cause a hemorrhage.
